I’ve just added a HighResolutionTimer class that uses OS-specific tricks like Windows MM timers or high-priority threads to run an accurate high-speed timer callback. Seems pretty good in my tests, accurate to less than 1ms, but would welcome feedback/testing!

FWIW - I just implemented it at 2ms accuracy on my older iPad Mini, playing two MIDI tracks ... so far it seems to work like a charm, it sounds very solid, and uses about 10% of my app's total cycles, and my app itself is only using about 30% of the available cycles on the old Mini.
